About this policy
Longevity Health Clinic is operated by Longevity Health Clinic PTY LTD (ABN: 55 698 352 902) (“LHC”). This policy explains how LHC manages personal information under the Privacy Act 1988 and applicable Australian Privacy Principles. It covers enquiries, assessments, care coordination and memberships provided by LHC. Independent treating providers may also have their own privacy policies.
Information LHC collects and holds
Depending on your interaction, LHC may collect identification and contact details; date of birth and location; health concerns, history, medications, allergies, lifestyle information and goals; referral letters and pathology results; consultation and care records; appointment and membership details; billing and payment records (card details are processed by our payment provider, ArtsPay, and are not stored by LHC); consent preferences; and correspondence or complaints. LHC limits collection to information reasonably needed for its activities.
How information is collected
LHC generally collects information directly from you through forms, consultations, telephone calls, email and the patient portal. With appropriate consent or another lawful basis, LHC may also receive information from your authorised representative, referring or treating practitioners and pathology providers. LHC will take reasonable steps to explain relevant indirect collection where required.
LHC generally seeks your consent to collect health information, unless the law permits or requires collection without it. Consent to information handling is separate from consent to treatment. You may ask questions, decline optional information or discuss withdrawing consent for future handling. LHC will explain any resulting limits on care.
Why information is used and disclosed
LHC uses information to answer enquiries, assess suitability, arrange and provide services, coordinate clinically appropriate testing and care, maintain records, manage bookings and payments, respond to complaints and meet legal obligations. Information may be used or disclosed for these purposes, related purposes permitted by law, with your consent, or where otherwise required or authorised by law.
Relevant recipients may include your treating doctor, nutritionist, movement practitioner, pathology laboratory, other clinicians involved in your care, authorised administrative staff and providers of hosting, clinical records, communications, payment and IT support. Disclosure is limited to what is appropriate for the recipient’s role. Regulators, insurers or legal advisers may receive information where there is a lawful basis. Family members and employers do not automatically have access to your records.
Marketing choices
Optional marketing is separate from care communications. Promotional email or SMS is sent only where you have opted in. You can withdraw that choice through the message’s unsubscribe option or by contacting LHC. Declining marketing does not affect access to care. LHC does not use health information for direct marketing without the consent required by law.
Storage and security
LHC uses the following systems to hold and manage information: ScriptedTech for clinical records and the patient portal, with clinical data stored in Australia; Google Workspace for email and documents; GoHighLevel for enquiry management, appointment communications and messages; and ArtsPay for payment processing. LHC takes reasonable steps to protect information from misuse, interference, loss and unauthorised access, modification or disclosure. These include individual staff logins, two-factor verification and limiting access to staff who need the information for their role. No system can be guaranteed completely secure.
Information is retained for clinical, administrative and legal needs, including applicable health record retention requirements. When it is no longer needed and retention is not legally required, LHC takes reasonable steps to destroy or de-identify it. A request to withdraw consent or close an account does not necessarily permit immediate deletion of clinical records.
Data storage and overseas handling
Clinical records are held by ScriptedTech, with clinical data stored in Australia. LHC also uses service providers including Google Workspace, GoHighLevel and ArtsPay to support its operations. Personal information handled through these services may be stored or processed outside Australia, including through hosting providers, payment partners and authorised support teams.
GoHighLevel identifies data-storage providers in the United States and service and support operations in India. Google Workspace uses international infrastructure; the location of covered data depends on the service and LHC’s configured data-region settings. ArtsPay identifies possible overseas handling in Singapore, the United States, Ireland and other European countries. The providers and locations relevant to your information depend on the services used.
Before disclosing personal information overseas, LHC takes reasonable steps required by Australian privacy law to protect that information. You can contact LHC at admin@longevityhealthclinic.com.au about its service providers and overseas handling.
Website information and cookies
Our website uses only the cookies and similar technologies needed to operate and secure the website and its inquiry and payment functions. These technologies may process technical information such as your IP address, browser, and device details. We do not use analytics, advertising cookies,s or tracking pixels on this website. You can manage cookies through your browser settings, although blocking necessary cookies may affect some website functions. Access and correction
To request access to your information or a correction, contact the Privacy Officer at admin@longevityhealthclinic.com.au, 0492 811 098 or Suite 45 Riva Office, 2/14 Edgewater Court, Robina, Gold Coast, QLD. LHC may verify your identity or authority before responding. LHC aims to respond within 30 days and will explain any delay. There is no fee to make a request or seek a correction. If a reasonable access charge is proposed, LHC will explain it in advance.
If access or correction is refused on a lawful ground, LHC will provide reasons where required and explain complaint options. If a correction is not made, you may ask LHC to associate a statement of your concern with the record. LHC will address requests to notify relevant recipients of a correction as required by law.
Privacy complaints
Contact the Privacy Officer with what happened and the outcome you seek. LHC aims to acknowledge complaints within two business days, investigate fairly and provide a response within 30 calendar days. If more time is needed, LHC will explain why and provide an update. You may use an authorised representative.
If unresolved, you may contact the Office of the Australian Information Commissioner at www.oaic.gov.au/privacy/privacy-complaints or 1300 363 992. The OAIC generally expects you to raise the matter with LHC first and allow a reasonable response period, usually 30 days.
General enquiries and policy updates
Where lawful and practical, you may make general enquiries anonymously or using a pseudonym. Identification and sufficient health information may be necessary to provide safe clinical services. This policy is available free of charge; contact LHC for another accessible format. LHC updates it when practices or legal requirements change and displays the effective date above.
