Privacy Collection Notice

This notice explains what happens to the information you provide when you send an enquiry to Longevity Health Clinic.
Who collects your information
Longevity Health Clinic is operated by Longevity Health Clinic PTY LTD (ABN: 55 698 352 902) (“LHC”). Contact the Privacy Officer at admin@longevityhealthclinic.com.au, 0492 811 098 or Suite 45 Riva Office, 2/14 Edgewater Court, Robina, Gold Coast, QLD.
What is collected and why
LHC collects the contact details, state, areas of interest and other information you provide in the enquiry form. Information about your health, goals or support needs may be sensitive health information. LHC uses it to respond to your enquiry, consider which services may be appropriate and arrange an assessment if you choose to proceed.
Please provide only a brief enquiry. Do not upload medical records or include detailed medical history in this form. Further health information will be requested through the clinic’s intake process when needed for your care.
Your choice
Submitting an enquiry is voluntary. LHC does not require this enquiry information under a specific law. If you do not provide the required contact details or information needed to understand your enquiry, LHC may be unable to respond or arrange an appropriate service. You may make a general enquiry without giving your name, where practical.
Who may receive the information
Relevant information may be accessed by authorised clinic administration staff and service providers supporting the enquiry system. If you proceed with care, information relevant to that care may be shared with your treating doctor, nutritionist, movement practitioner and the pathology laboratory involved in your testing. These recipients receive information relevant to their role; an enquiry does not automatically authorise circulation of your full health record to everyone.
Website analytics and advertising
Our website uses Google Analytics to understand how visitors use the site and improve their experience. Google Analytics may collect technical and usage information, including browser and device details, pages viewed and website interactions. We do not currently use Meta Pixel. Please see our Privacy Policy for more information about how we handle personal information.
Data storage and overseas handling
Clinical records are held by ScriptedTech and stored in Australia. LHC also uses technology and payment providers including Google Workspace, GoHighLevel and ArtsPay. These services may involve overseas storage, processing or access, including in the United States, India, Singapore, Ireland and other European countries. Privacy rights and questions
The Privacy Policy explains how LHC holds and handles information, how to request access or correction, and how to make a privacy complaint. Contact the Privacy Officer using the details above if you have questions or wish to discuss withdrawing consent to future handling. Withdrawal may affect the services LHC can provide and does not remove lawful recordkeeping obligations.
The enquiry form is not monitored for emergencies. Call 000 in an emergency.
